    Breast implant surgery through the belly button is one of the most exciting developments since the original breast augmentation procedure.  This surgical technique allows patients to circumvent the most common patient complaint, which is surely the possibility of postoperative scarring common to every other kind of implant placement surgery.  Undergoing breast enlargement using a naval incision does have its drawbacks and limitations, but offers patients an alternative approach which is sure to satisfy many of their concerns about the augmentation process.

    The technical medical name for this operation is transumbilical breast augmentation, often abbreviated TUBA.  The procedure utilizes a small, completely hidden incision inside the natural folds of the belly button to gain entry to the breast tissue for implant placement.  There are no visible incisions made anywhere in the anatomy and there will be no definitive evidence that augmentation has ever occurred. This procedure is unique in these aspects, as all other types of breast implant placement do use external and often noticeable incisions in one of 3 common locations.  Before detailing the TUBA approach, let’s look at the more common alternatives…

    Periareolar breast augmentation is the most common in the USA and in many other areas of the world.  This approach uses small semi-circular incisions in the bottom of the areola to place the implant directly into the breast.  This technique can be used with saline or silicone implants, although silicone will require a much longer incision and will not work well for every patient, especially those with diminutive or light colored areolae.  Scarring is often visible on the lower portion of the areola and can be extremely disconcerting if the scars are prominent.  Additionally, being that this technique cuts into the actual breast mound, there is increased risk for complications including nipple sensitivity issues, breastfeeding issues and infection.

    Inframammary breast augmentation is very common and is performed using a crescent shaped incision placed in the breast fold, under the breast mound.  This inframammary crease is located exactly where the mammary attaches to the body, under the bulk of the breast.  This is the original approach to augmentation and can be used with any type of implant prosthetic.  Risk of infection and breastfeeding difficulties are decreased using this approach, as is the chance for diminished nipple sensation.  However, this technique also has a serious downside.  First, the scars are usually the worst with this method of placing implants and will be easily noticed when looking at the underside of the breast.  Worse still is the fact that cutting into the breast fold accelerates the natural degenerative processes which cause ptosis (sagging) of the breast mound.  This can cause early overall breast degeneration or can result in various implant based complications including double bubble, or more commonly, bottoming out of the prosthetic.

    The transaxillary breast augmentation technique was revolutionary in that it was the first approach to successfully place implants using incisions off the breast mound.  This method enters the body through a fine incision in the underarm.  The incision can be hidden in a natural fold, although it is still often visible years later when the axilla is exposed.  This operative procedure also allows patients to reduce the chance of experiencing complications relating to breastfeeding and nipple sensitivity.   The transaxillary technique does not have degenerative effects on the breast fold or other mammary tissues.  This surgery can be performed using any type of implant, although it works best with saline or smaller sized silicone prostheses.  The downside is the possibility of nerve damage or injury to the lymph nodes in the underarm region and a greater chance for implant migration away from the intended site of placement.  Of course, the scarring is also a consideration for many women, especially dancers, models and those who often lift their arms, exposing the incision locations.

    Finally, we get to TUBA.  The transumbilical approach allows women to reduce all the chances for breastfeeding and sensation complications, as well as avoid damaging the breast structure or enacting noticeable scarring.  However, the potential downside of the procedure is that is can currently only be utilized with saline breast implants.  Additionally, the technique is not indicated in all women, since individual anatomical concerns may preclude some women as ideal candidates.  However, these events are rare and most women will truly benefit from this option when considering surgical breast enlargement.

    In conclusion, the transumbilical approach to breast implant placement gives women one more choice to achieve the look and feel they desire without risking many of the usual negative possibilities which are associated with more traditional prosthetic placement.     The ReFirme or Polaris-Laser Lift is one of the most popular FDA-approved nonablative procedures for maintaining a youthful appearance by skin tightening. Unlike Thermage, it uses a laser combined with radio-frequency energy and pulsed light. The last method of skin tightening involves laser skin resurfacing. The older methods include the non-fractionalised erbium or CO2 lasers. The newer safer methods include erbium with profractional technology, the Fraxel or the gold standard Active Fx fractionalised CO2 laser. Many doctors, including myself believe a combination of treatments (surgical and nonsurgical) get the best tightening results. The Refirme (used to be called the Polaris) is probably my favourite method as it is not especially painful, has a combination rejuvenation laser, does not cause later skin damage and has a lot of experience in the cosmetic market.

    Why do you like the Polaris-Laser Lift over other RF devices?

    The box says “this advanced system incorporates three forms of energy to improve skin texture and skin colour”, but I technically like the Polaris RF because it uses bipolar radiofrequency, which has been shown to be safer and more effective than monopolar. Unlike less controllable and less safe monopolar, bipolar RF does not travel through the body and this means each treatment can be customized through the system’s computer to control the depth and degree of the heating process; hence the technology can focus energy to a specific depth of skin for more accurate treatment. Newer tripolar devices may even prove to be better again. I also like it as the RF can cause the collagen to contract and tighten over a period of time and the combination diode laser can help to reduce other problems, such as redness and brown pigmentation blotches. The procedure is also rapid and bloodless. It is usual to have four sessions of treatment are performed separated by three weeks. It is favoured by many celebrities including Madonna and Felicity Kendall.

    How does the Polaris RF system actually work?

    Nobody really knows how this system works but we assume radio-frequency (RF hereinafter) passes electrical energy through the tissue and resistance produces heat within the deeper dermis. This bundle of thermal energy can be accurately controlled by computer evenly to three-dimensional volumes of collagen tissue at controllable depths. This energy causes immediate collagen denaturation of the triple-helix, which is thought to be the mechanism for immediate tissue contraction. There is also subsequent new collagen formation (neocollagenesis) over the next month, which further tightens the dermal tissue and reduces wrinkles.

    What are the benefits of Polaris-Laser Lift over other procedures and surgeries?

    For a start, it is totally non-invasive. This means there are no incisions and no evidence to date of later tissue damage. There is no bleeding or bruising and the risk of scarring is minimal. A level of control can be achieved with the Polaris that is difficult with other treatments. It utilizes three different forms of energy to reach multiple skin problems. This means we can see an improvement in wrinkles, sagging and colour. There may be areas of slight swelling or redness that quickly resolves over a few days. Make-up may be applied immediately. You may resume all normal activities immediately. The full effect of your treatment develops over a few months following the procedure and can be expected to last for several months or years. As with any cosmetic procedure, the results can be expected to fade gradually over time, at which time the procedure can be repeated if desired.

    Is it painful?

    A Polaris-Laser Lift can be performed with topical anaesthetic cream applied about an hour before your treatment. Some patients do not require anaesthetic cream due to the cooling created by the stream of cool air that hits the skin during treatment. When the machine is pulsed, patients feel brief, heat in the treatment site followed by the immediate cooling effect of the air spray. For those patients who are sensitive, the topical anaesthetic cream is recommended. 
    What kind of results can I expect?

    As with all RF devices, this is difficult to estimate. Some patients see results immediately. However, for most patients the results are gradual over a three to six month period as the skin tightens and more new collagen is gradually formed. Depending on the area treated, the skin laxity and your desired results, multiple treatments may be required. Skin will continue to age as it has in the past and eventually you may wish to have another anti-aging procedure. Most patients will see fine lines and wrinkles around the eyes and mouth fill in, while the deeper frown lines and grooves are softened. Facial skin and the skin along the neck can be tightened providing a mild, non-surgical browlift or facelift effect. This tightening effect can be used to firm up loose or sagging tissue in other parts of the body. This same deep Polaris tissue heating may also have benefits for treating acne, acne scarring and stretch marks. Redness and brown blotchiness also gradually improve two to three months after the treatments are complete. The worldwide safety record of this procedure has revealed very few adverse effects thus far.

    How much does it cost?

    According to the American Society for Aesthetic Plastic Surgery Report, in 2008 195,104 cosmetic eyelid surgeries were preformed in the United States, making it the third most popular plastic surgery performed. An eyelid tuck, medically termed a blepharoplasty, is a type of cosmetic surgery whose purpose is to remove sagging, unwanted skin, bulges and hanging skin from around the eye. Excess skin, puffiness, and crow’s feet can be eliminated with an eyelid tuck.

    There are four types of eyelid tuck available today: upper eyelid, lower eyelid, transconjunctival and Asian eyelid blepharoplasty. Upper eyelid procedures are used to decrease the amount of surplus skin on the upper eyelid and to cut down puffiness in the inner corners and middle of upper eyelids. Upper eyelid tucks aesthetically improve the shape, depth and position of the crease.

    Lower eyelid tucks remove the bags and puffiness resulting in excess fat deposits and skin under the eyes. Younger people have no excess skin on the lower eyelids, and therefore, only have excess fat. Their surgeons may decide to use the transconjunctivial blepharoplasty procedure, where an incision is made inside the lower eyelid and no visible scar is left. Asian eyelid tucks are known as double eyelid procedures and their purpose is to create a natural appearing eyelid crease, without changing the ethnic look.  All of these procedures can be used together or separately.

    Eyelid tucks take, on average, one to three hours to perform. Patients are usually given local anesthetic and sedatives during the procedure, though some surgeons prefer to use a general anesthetic. After the procedure is performed, the eyes are lubricated and bandaged. Pain medication is normally prescribed to counteract the affects of the procedure, including tightness and pain. Stitches are removed anywhere from two days to a week post-operation, depending on the patients healing progress.

    Eyelid tucks improve self confidence of the patients and can drastically improve the appearance of the eyes. As with all cosmetic procedures, the results are only as good as the surgeon.     Some years ago, a lengthy article was published by the BBC on the vagaries of the baby boomer generation.  It was a damning critique of a generation hell bent on wrecking the planet, along with most of its social standards.

    So how has the baby boomer species ruined the World?  They stand accused of owning homes that are too big; gas guzzling cars that are too big and require to be driven too far each day to work and home again; taking too many holidays too often to exotic locations; over spoiling the grandchildren; causing the divorce rate to rocket; not to mention sexually transmitted diseases rumored to be rooted in baby boomer ‘free love’ from the sixties onward; and generally being a scourge on an otherwise relatively even keeled planet!

    Although one or two boomers have made a half hearted attempt to protest their innocence and declare that their society is simply more transparent and that the World has always struggled with poor moral standards, in our modern society we tend to forget that the pre-baby boomer years were comparatively free from the alcohol and drug related problems we now have to deal with on a daily basis.  Apart from the occasional ‘drunk’ who would be a local scandal, the thirties and forties were lived in a cocoon of respectability until we baby boomers came along and exploded our values – or lack of them – on society; causing moral havoc everywhere.

    To add insult to injury, the baby boomer generation continued its sexual freedom motif at all cost and instead of putting a curb on its sexual activities, spent squillions on research to sponsor the ultimate birth control – the Pill!!  Hooray! Contraception was the new age weapon against unwanted pregnancy and funded with almost the same enthusiasm as the space race.  Sexually transmitted diseases were warded off with condoms, brought to modern sophistication to make them more comfortable and marketed to exhaustion: with relatively foolproof protection at its fingertips the baby boomer set thumbed its nose at the rest of the World and carried on doing what it did best…

    But – we baby boomers have also blessed the World with some ‘good stuff’.  Let us not forget space travel; plastic surgery; computer technology; alternative medicine; the mobile phone; the morning after pill; food hygiene; seat belts; sat nav; disposable diapers; the hundreds of cures that have been brought to people with horrible diseases and disabilities: baby boomer technology is not all bad…

    Baby boomers are thought to be single-handedly responsible for the breakdown of family values in the new Century.  With the escalation of the divorce rate, the onset of the ‘new family unit’ became something the World had to cope with; stepsons and daughters having to build trusting relationships with step fathers and mothers; children abandoned while their natural fathers or mothers sought new relationships elsewhere; the old concept of ‘making your bed and lying in it’ out the window; to be replaced by the ‘take what you can get out of life because everyone else does’ attitude that is more typical of baby boomers than loyalty, fidelity and ‘staying together for the sake of the children’, so popular in pre-WW2 days.

    ‘Consumerism is all’ to many of the baby boomer generation; unsurprising when you consider the product standards on offer.  Baby boomers have on their doorstep an array of consumer goods the like of which the World has never seen: stand in any department store on Christmas Eve or Hanukkah in New York or London or Paris and savor the aroma of melting credit card plastic, mingling with the scent of new fabrics, perfume and expensive leather to know just how baby boomers have changed the face of religious festivals Worldwide!  We just don’t know when to stop, it seems…

    We like labels; we like our youthful image and we do not accept our advancing years as previous generations have graciously accepted theirs – bring on the Botox and the liposuction -and charge it!

    As women continue to be liberated and strive for sexual equality; as gays fly out of the closet at the rate of knots and as America invites its first African American president to make a cameo appearance on ‘The Simpsons’, it is obvious that the boomer generation accepts nothing as impossible.

    The greediest and most consumerist generation the World has ever seen is by nature deaf to criticism so there is little point in harping about boomers leaving the planet in poor condition!  Knowing baby boomers, it will be passed on tagged ‘as seen!’

    Baby boomers continue to procure the best that money can buy, despite dire warnings of recession; global warming; the death of the planet; disease; pestilence and the collapse of the family unit.  Love us or hate us, we are simply incorrigible…!

Older women battle age and gravity, chasing after the perky, firmer breasts of their youth. Younger women seek sex appeal, and often the perfection and symmetry nature didn’t award them. If you are considering breast augmentation surgery, you have three different types of implants to choose from.

    Saline implants are the most used in breast implant history. The FDA banned silicone gel implants in 1992 after women suffered serious health problems, leaving only saline implants.

    Saline implants have been considered the safest because they are filled with a saltwater solution. If an implant should leak or rupture, the saline will be absorbed by the body and passed out through the urine. This implant allows the surgeon to make a smaller incision (less scarring) because the saline is added after the implant is already in the body. However, a saline implant has the reputation of not feeling or moving like a real breast.

    A silicone gel implant better mimics the movement of a real breast because it is filled with the soft gel. The FDA banned them for 14 years, during which clinical trials studied and analyzed them for health risks resulting from silicone leakages. In 2006, the FDA finally approved them for unrestricted use. But are they really safe?

    There is still one major disadvantage of the silicone gel implants: it is extremely difficult to detect a rupture or leak. Even your surgeon can’t be positive without utilizing an MRI. That is why the FDA recommends women with implants have an MRI every two years. MRIs are expensive and not covered by health insurance, so they should be factored into the total cost of the breast implants.

    A major advantage of silicone gel implants is that the FDA has studied and analyzed them more than any other medical device in American history. Nowadays, some consider them even safer than saline implants.

    You may not even have heard about the cohesive gel implant – also called the gummy bear implant. This implant is a solid gel mass with an overall gummy texture. There is no liquid inside it. It won’t leak even if it’s cut in half! You will simply have an implant cut into two pieces. A major drawback to this implant is that because it is a solid mass, the surgeon has to make a larger incision (meaning a larger scar) to insert the bigger implant into the breast.

    The biggest disadvantage of the cohesive gel implant is that the FDA has not approved them for use in America. The rest of the world has used them for almost 20 years, but the FDA feels they need more study. If you are seriously interested in having the cohesive gel implants, you will need to locate a plastic surgeon the FDA has approved for its clinical trials.

    For most women who live their lives in bodies they are not satisfied with, the humiliation of small or deformed breasts is too much to live with if it can be helped. Maybe the women were teased growing up, or some other painful memory haunts them. No matter what the reason, the desire to correct their flaws is strong and unending. For these women, cosmetic surgery far outweighs the risks of surgery. If this scenario applies to you, think before acting. Before going under the knife, you should know the risks.

    As with any surgery, there is a risk of complications from anesthesia, bleeding, infection, slow healing, and scaring. The risks of those complications happening with a breast augmentation surgery aren’t any higher than with any other type of surgery, so long as the surgeon is good, you have followed all the pre-op instructions and proper sterilization procedures are followed.

    Breast Augmentation specific risks are many, though relatively rare. Capsular contracture, that is, hardening of the scar tissue surrounding the implant, is arguably the most common complication. Breast droop, deflation, displacement and rupture of the implant is also not unheard of. Wrinkling of the implant is more common, as well as more noticeable, in women with naturally sparse breast tissue. Skin irregularities, such as puckering, can occur with breast augmentation surgery. Desensitized breasts, nerve damage and complete nipple numbness have also been reported.

    More serious risks include hematoma, which is pooling of clotted blood in the breasts. Seroma, another serious risk, is the pooling of watery blood around the implant. Though very rare, symmastia is another risk to consider. Symmastia happens when the implants move and merge into one mass.

    Last year in the United States, approximately 60,000 women choose to have a Breast Lift (or Mastopexy), a surgery that raises and reshapes breasts that have sagged from pregnancy, significant weight change, or the natural process of aging. A number of these women, wanting more breast volume, had augmentation with saline or silicone gel implants at the same time.

    Augmentation Mastopexy is a complex surgery that challenges even the most skilled and experienced plastic surgeon, who must deal with the opposing forces required to make breasts fuller (by stretching the skin with implants) yet firmer (by tightening the skin with a breast lift).

    For many years, like my colleagues, I would draw a complex pattern on the breast, remove some of the excess skin, and then tailor tack these tissues together. I would do the same on the other breast, and then trim excess skin multiple times to provide maximum tightening and make sure that the breasts were still symmetrical.

    About six years ago, when I was performing an Augmentation Mastopexy, the proverbial light bulb went off: If I first tightened and tailored the breasts with surgical skin staples, perhaps I could identify the total amount of excess skin to be removed and previsualize the final breast shape and symmetry before I made an incision.
